CASE MANAGEMENT
The Sickle Cell Disease Association of America-Mobile Chapter’s Case Management Program helps individuals with Sickle Cell Disease access a wide range of medical, social, educational, and community services in the area. The program offers support for clients and their families in coping with the financial and emotional challenges of the disease. Case managers assist with resources related to housing, transportation, childcare, medications, insurance, education, career assistance, and psycho-social issues. Additionally, the program provides financial aid to help clients purchase necessary medications and treatment equipment.

SCDAA-MC Demand Response Transportation Program
The Sickle Cell Disease Association – Mobile Chapter is committed to eliminating transportation barriers for individuals affected by Sickle Cell Disease. Our Demand Response Transportation Program provides no-cost, flexible, and accessible transportation across all nine counties in our service area. This initiative ensures that those in need can attend medical appointments, access essential services, and participate in community support programs.

Eligibility Criteria
To qualify for transportation services, individuals must meet the following criteria:
Must be diagnosed with Sickle Cell Disease. Individuals w/ Sickle Cell Trait are not eligible.
Must reside within the nine-county service area.
*** Transportation is available for the following purposes: ***
- Medical appointments (doctor visits, specialist care, treatment centers, therapy).
- Prescription pick-ups and pharmacy visits.
- Support groups and health-related community events.
Caregivers and parents of minor patients may request transportation.
Scheduling & Reservations
Advance Booking Required:
Rides must be scheduled at least 48 hours in advance to ensure availability.
Same-day requests may be accommodated in emergency situations, subject to availability. Standing (recurring) appointments can be scheduled in advance.

Additional Program Policies:
Companion Policy: One caregiver, parent, or support person may accompany the rider if needed.
Emergency Accommodations: While this is not an ambulance service, we strive to assist with urgent, non-emergency medical transportation needs.
No-Show & Cancellation Policy
Riders must provide at least 24-hour notice for cancellations.
Repeated no-shows may result in temporary suspension from the program.

Services are based on available funding.
AFTER-SCHOOL TUTORIAL PROGRAM
The SCDAA-MC provides educational assistance to students with Sickle Cell Disease for grades 1-12, helping them keep up with their studies despite frequent absences due to hospitalizations and health issues. Certified teachers are available to support these students through a tutorial program, which operates Monday through Friday from 3:30 pm to 6 pm, during the school year. Enrollment is open throughout the year, and transportation is provided.
SUMMER ENRICHMENT PROGRAM
The Summer Enrichment Program is for children with Sickle Cell Disease and continues their education during the summer. It includes day-long educational field trips and activities that enhance their learning. Participants engage in art, cultural activities, and programs focused on college and career preparation. POSITIVE PERSPECTIVES (SUPPORT GROUP)
Positive Perspectives is a supportive forum where clients and their families can discuss topics of interest and share their experiences of coping with Sickle Cell Disease. Meetings are held quarterly and complemented by additional activities and events. For more information, please contact SCDAA-MC.
